This program assists vulnerable individuals and families, like those who are low-income, seniors, people with disabilities, and veterans, with personalized case management to access benefit programs. Individuals may need support in accessing things like housing, healthcare, food security programs, or mental health support. MSCF recognized the importance of removing barriers to stability, and thus awarded At Liberty Services two $1,500 grants to assist them in their mission of restoring dignity, building independence, and creating lasting change; One from the Foundation’s unrestricted Money in Motion Fund, and one from the Plainville Community Fund.
